N.B. All forms have the new "electronic forms" wording where appropriate.

These have the same website address as the old forms (so your bookmarks will continue to work, but refresh the page if necessary). The words "Form amended by SI 2020/1072 for use from 1 December 2020" appear at the bottom of the amended forms so that you can quickly ensure that the new wording has been used: this wording does not affect the validity of the forms any more than the name/address of a company selling the forms would.
