From 3/11/08, when most of the changes made by the Mental Health Act 2007 come into force, new statutory forms will have to be used.

England

In England, forms can be purchased through the NHS Forms Contract, or otherwise designed to comply with the Mental Health (Hospital, Guardianship and Treatment) (England) Regulations 2008.

If anyone has forms which they'd be willing to upload to this site, then please send them in (see How to help the site). Note the errors in the Regulations listed on the CSIP website (see link below).

Wales

In Wales, forms should comply with the Mental Health (Hospital, Guardianship, Community Treatment and Consent to Treatment) (Wales) Regulations 2008. The Welsh Assembly Government plans to produce forms free of charge.
