Mental Health Act 1983 Statutory Forms

From 3/11/08, when most of the changes made by the Mental Health Act 2007 come into force, new statutory forms will have to be used.

England

In England, forms can be purchased through the NHS Forms Contract, or otherwise designed to comply with the Mental Health (Hospital, Guardianship and Treatment) (England) Regulations 2008. Note that some errors in the regulations have been corrected by the Mental Health (Hospital, Guardianship and Treatment) (England) (Amendment) Regulations 2008.

Wales

In Wales, forms should comply with the Mental Health (Hospital, Guardianship, Community Treatment and Consent to Treatment) (Wales) Regulations 2008. The Welsh Assembly Government plans to produce forms free of charge.
