R (P) v HM Coroner for the District of Avon [2009] EWCA Civ 1367
In this inquest to which Article 2 applied (suicide in prison) the Deputy Coroner misdirected the jury because she did not properly explain to them that, if they returned a verdict of suicide or accident, they could also append a narrative about the circumstances of the accident. However, in the circumstances, the verdict was not quashed.
